Moving Home Minnesota (MHM)
Our Moving Home Minnesota (MHM) team creates opportunities for individuals to move from qualified institutions to their own home in the community. Our MHM team will assist the person served in
developing a transition plan to relocate to the community, implement the transition plan, and purchase items necessary to establish the individual in the community (up to $3,000 in furnishings, supplies, and moving expenses).
Services include

Connecting individuals with housing opportunities

Assisting individuals with obtaining monthly income through Social Security or General Assistance

Assisting individuals with certification of disability through the State Medical Review Team (SMRT) and Social Security
-
Setting up services for individuals in the community such as Medicaid Waivers, PCA, PT/OT, etc.

Providing funding for security deposit, moving expenses, furniture, and household items for when an individual transitions to the community

Continued support for up to one year after individual has transitioned to the community including:

​Demonstration Case Management Services

Comprehensive Community Support Services designed to assist individuals in establishing supports in the community, maintaining housing, and household management

Home modifications to make a person’s home handicap accessible

Additional supplemental services not eligible to individuals through other programs such as providing funding for membership fees for exercise classes or health club membership fees up to a year after moving​

Eligibility:
Individuals must be enrolled in Medical Assistance (MA), must have continuously resided in a Qualified Institution for 60 days, and must transition to a qualified community residence.

Qualified Community Residence:
-
A home owned or lease by the member or the members family
-
An apartment with an individual lease and living areas over which the member or member’s family has domain and control
-
An assisted living that providers an apartment with separate living, sleeping, bathing, and cooking areas, lockable entrance and exit

A home in a residential setting in which no more than four unrelated individuals live
