Mental Health Case Management

Independent Living Placement’s Adult Mental Health Targeted Case Management team provides person-centered, recovery-based services to adults with a diagnosed serious and persistent mental illness (SPMI). Our targeted case managers provide supports to
individuals experiencing mental and/or chemical health needs as well as those with a history of trauma, with a focus on helping them work on recovery and the ability to live independently in the community.

ILP’s mental health case managers help individuals identify goals and services, refer and link individuals to needed resources to achieve their goals, and coordinate care between treatment team providers. Our case managers work to build relationships based on meeting individuals where they are and providing encouragement for recovery and goal achievement. They place a high emphasis on fostering hope, courage and resiliency, and use a strengths-based approach when regarding individuals’ life situations.

Some examples of what a mental health case manager can assist with:

  • Identifying strengths and needs

  • Person-centered goal planning

  • Finding housing and ensuring stability

  • Finding resources for psychiatric care, therapy, and chemical health programs

  • Assistance with obtaining  medical and mental health care coverage

  • Crisis planning

  • Employment/educational support

  • Support applying for and monitoring financial assistance programs

  • Support to overcome daily life challenges

Some examples of recent goals achieved through working with our mental health case management team: helping a single mother find affordable housing, connecting an individual to mental health therapy and medication management services, accessing the Community Access for Disability Inclusion (CADI) Waiver for a wider range of supportive services, completing benefit eligibility forms and MNsure applications for healthcare coverage, helping find energy assistance. 

Referrals to our program are sent via Ramsey County after an individual has been referred by a community mental health provider. If an individual is not connected to a community provider, they can contact the Ramsey County Welcome Center at 651-266-7890 to discuss a possible referral to case management services.
